On completion of this continuing professional development (CPD) activity, participants will be able to: • describe advanced care planning and its relationship to person-centred care • identify who is responsible for initiating advance care planning discussions in their nephrology department • identify the benefits of advanced care planning • recall conversation triggers for raising the topic of advanced care planning.
