The first session of systemic therapy is key to engaging and understanding the difficulties that the family is bringing to therapy. It can be a challenging session because of the uncertainty the family is experiencing at having to speak about difficulties that have usually been present for some time. Similarly, the therapist can be feeling much the same way with the uncertainty of what will be discussed and how will it be managed. This chapter is designed to support the first session process for both the therapist and the family by providing a structure to work within that maintains a contained and secure base for everyone. The chapter includes step by step guidance, including questions to utilise, and tips on how to attend to the process as well as the content of the session. Before presenting the guidelines, the reader is reminded of some of the key concepts in systemic thinking as a theoretical foundation for the practical focus that follows.
