Valuing Digital Health: Using Stakeholder Preferences to Inform the Evaluation of Digital Health Maturity

Digital maturity in healthcare reflects an organisation’s ability to use technology to generate value through innovative solutions. As digital health adoption accelerates, robust approaches are required to evaluate clinical, economic, and stakeholder outcomes. Traditional economic evaluation frameworks often fail to capture the dynamic, user-centred, and system-wide impacts of these technologies. This thesis examines how value is defined and assessed for digital health, with a focus on artificial intelligence (AI) in oncology. A scoping review identified key challenges in existing AI evaluation frameworks, including an overreliance on accuracy metrics rather than clinical effectiveness, and limited consideration of implementation and equity. A critical gap emerged: the need for value assessments that incorporate stakeholder perspectives, particularly within the Australian healthcare context. To address this gap, the thesis applies stated preference methods to elicit stakeholder values. Empirical evidence from two discrete choice experiments and a best–worst scaling survey quantifies stakeholder preferences for AI in oncology and national digital health priorities in Australia. These findings provide practical guidance for aligning digital health strategies with user values. By integrating stakeholder perspectives into value assessment, this research supports more responsible, effective, and equitable implementation of digital health technologies across healthcare systems.
